Responsibilities:

• Conduct comprehensive assessments and develop individualized care plans for residents in accordance with federal and state regulations
• Collaborate with interdisciplinary teams to ensure seamless care transitions and achieve optimal outcomes
• Coordinate and facilitate care planning meetings, educating residents, families, and care team members on care plans and progress
• Utilize electronic medical records to document accurate and timely assessments, care plans, and progress notes
• Stay current with industry developments, best practices, and regulatory requirements, applying this knowledge to improve care delivery and outcomes
