Job summary

Laurelwood Healthcare Center in Maryland is seeking an MDS Coordinator to manage the Minimum Data Set assessment process in accordance with federal and state requirements. You will collaborate with nursing, therapy, social services and other care team members to ensure accurate documentation and compliant care plans.

Qualifications

• Experience in a skilled nursing or long-term care setting preferred.
• Knowledge of the MDS/RAI process and regulatory requirements.
• Strong clinical assessment and documentation skills.
• Proficiency with electronic medical records and standard computer applications.

Responsibilities

• Coordinate and complete MDS assessments according to required schedules and regulatory guidelines.
• Review resident medical records and documentation to support accurate MDS completion.
• Conduct resident interviews and gather information from residents, families, staff, and other sources.
• Coordinate with interdisciplinary team to obtain information for MDS assessments.
• Identify changes in resident condition that may require additional assessment or care plan updates.
