Job summary

Windsor Elk Grove Care and Rehabilitation Center in Elk Grove, CA is seeking a Full-Time Minimum Data Set Nurse (MDS-RN) to join our team. You will lead the MDS process and collaborate with caregivers to ensure accurate resident assessments and compliant care planning.

Responsibilities include coordinating the MDS for all residents, submitting data per state and federal regulations, and maintaining data integrity while supporting Medicare participation and quality resident care.

Qualifications

• 1-3 years of MDS experience.
• Nursing degree from an accredited college.
• Current and unencumbered RN license in California.
• Knowledge of clinical standards, regulations, and Medicare participation.

Responsibilities

• Assisting in the completion of the MDS assessment for all residents.
• Coordinating the MDS process, ensuring accurate and timely submission per regulations.
• Collaborating with interdisciplinary teams to gather data and support care planning.
• Maintaining accuracy and integrity of resident information by ensuring data is entered correctly.
• Monitoring resident status and changes that may require updates to MDS assessments.
• Assisting with regulatory compliance governing MDS and Medicare participation.
