Job summary

Socket.dev is seeking an MDS Coordinator - RN to ensure timely, accurate MDS assessments and reviews in a long-term care setting. You will coordinate care plans and support documentation to maximize appropriate reimbursement while maintaining high standards of care.

The role requires RN licensure with experience in long-term care nursing and familiarity with Medicare/Medicaid processes. Location is WV with on-site work and competitive benefits.

Qualifications

• RN licensure and completion of an accredited School of Nursing.
• Minimum of 1 year of long-term care nursing experience.

Responsibilities

• Complete MDS treatment assessment tool timely and accurately.
• Review care plans and ensure delivery aligned with patient needs and standards.
• Manage Medicare/Medicaid case-mix documents for reimbursement.
• Conduct concurrent MDS reviews to optimize RUG categories.
