Job summary

Continuing Healthcare Solutions in Ohio seeks a qualified MDS Coordinator to manage MDS assessments, ARDs, and care plan alignment. You will coordinate with the interdisciplinary team to ensure regulatory compliance and timely documentation.

The role requires an Ohio RN or LPN license with MDS experience, and familiarity with restorative programming and ICD-9 coding is required. Excellent organizational and teamwork skills are essential.

Qualifications

• Must be a Registered Ohio RN or Licensed Practical Ohio LPN with MDS experience.
• Familiarity with Restorative programming and ICD-9 coding required.
• Strong regulatory compliance knowledge and teamwork skills.

Responsibilities

• Review residents admitted and compare orders with each resident’s plan of care.
• Schedule routine MDS assessments (initial, quarterly, annual).
• Schedule ARD for routine MDS assessments with input from the Interdisciplinary Team.
• Schedule status change assessments as instructed.
• Complete Re-entry and Discharge tracking MDS within 7 days.

Experience and Skills

• Review residents admitted and compare physician orders with eachresident’s plan of care to assure appropriate implementation of thephysician’s plan of care*
• Schedule routine MDS assessments (initial, quarterly, annual) tomaintain compliance with regulatory time frames*
• Schedule ARD for all routine MDS assessments, following the mostcurrent RAI User Manual instructions, with input from theInterdisciplinary Team*
• Schedule change in status assessments as instructed*
• Complete Re-entry and Discharge tracking MDS within 7 days of the event.

Candidates must be a Registered Ohio RN or Licensed Practical OhioLPN with previous MDS experience. Familiarity with Restorativeprogramming and coding, at minimum ICD-9 is also required.
