Job summary

Regency Integrated Health Services is seeking a Care Management Professional to coordinate the RAI/MDS process within a Bastrop, TX facility. You will ensure accurate assessments, proper documentation, and timely reporting while complying with all payer requirements.

You will work closely with the interdisciplinary team to optimize care plans, manage ARDs, and monitor MDS schedules to support appropriate reimbursement and high-quality resident care.

Qualifications

• Graduate of an accredited school of nursing with current RN or LVN licensure.
• Ability to coordinate MDS/RAI processes per CMS guidelines.
• Strong documentation and reporting skills.

Responsibilities

• Coordinate the Resident Assessment Instrument (RAI) process for accurate, timely resident assessments.
• Collaborate with interdisciplinary team to develop and manage care plans.
• Maintain schedule and ARD references for MDS assessments and ensure compliance.
• Audit MDS process, validate coding, and report data integrity issues.
• Communicate with Regional Care Management Specialist and Business Office on case management and reimbursement impacts.
