Job summary

Brooks Rehabilitation seeks an experienced registered nurse to coordinate and oversee all MDS and resident care assessment processes at the University Crossing facility in Jacksonville, FL. You will ensure compliance with federal and state requirements and certify timely completion of MDS assessments.

The role requires Florida RN licensure with 3+ years in skilled nursing, BLS certification, and preference for BSN/CRRN; formal collaboration with a multidisciplinary team is essential.

Qualifications

• Must be licensed RN in Florida with at least 3 years in skilled nursing.
• BLS certification is required.
• BSN or CRRN preferred.

Responsibilities

• Coordinate and oversee all functions of the MDS and resident care assessment process.
• Ensure compliance with federal and state requirements and certify timely completion of MDS assessments.
• Schedule or oversee MDS assessments to assure timeliness and accuracy.
• Participate in interdisciplinary team and rehabilitation conference meetings.
• Maintain confidentiality and promote patient safety, infection control, and quality care.
• Communicate effectively with residents, families, and staff; uphold Residents’ Rights policies.
