Job summary

Brooks Rehabilitation is seeking an experienced Registered Nurse to coordinate and oversee MDS and resident care assessments at the University Crossing location in Jacksonville, FL. The role ensures compliance with federal and state requirements and timely completion of MDS processes.

The ideal candidate has Florida RN licensure with skilled nursing experience and a BSN (or related degree) with CRRN preferred, working Monday–Friday in a full-time day shift.

Qualifications

• Must be an RN licensed in Florida with skilled nursing experience.
• BLS certification required.
• BSN or related degree and CRRN preferred.

Responsibilities

• Coordinate and oversee MDS assessments for timely completion.
• Ensure compliance with federal and state requirements.
• Maintain confidentiality and adhere to patient rights policies.
• Communicate effectively with interdisciplinary team and families.
