Job summary

Ecumen is seeking a Nurse Assessment Coordinator to oversee the RAI process, including MDS and care plan development. You will work with an interdisciplinary team to ensure timely assessments, regulatory compliance, and PDPM guidance. The role supports a remote work option and Minnesota operations.

Requires RN licensure in MN, Associate's Degree in Nursing, and 2 years of related experience. Preferred is a Bachelor's in Nursing and RAC-CT certification.

Qualifications

• Associate's Degree in Nursing and MN RN license are required.
• Minimum 2 years related experience in a healthcare setting.
• Familiarity with HIPAA, safety policies, and regulatory guidelines.

Responsibilities

• Coordinate the RAI process including MDS, Care Area Assessment, and care plan development.
• Collaborate with interdisciplinary team to ensure timely assessments and plan updates.
• Ensure compliance with state and federal guidelines and Medicare requirements.
• Provide oversight of assessment completion and transmission to national repository.
• Analyze MDS-based Quality Measures and participate in QAA/QAPI committees.
• Educate staff and ensure timely completion of supporting documentation.
