Job summary

Ecumen is seeking a Nurse Assessment Coordinator (NAC) to oversee the RAI process, including MDS assessments and care plan development. This role collaborates with nursing, therapy, dietary, and social services to ensure compliant, timely documentation and ARD compliance.

The NAC serves as the PDPM expert and supports Medicare requirements across the care continuum. This .8 FTE position offers remote flexibility and a targeted annual pay range of $68,000–$76,000.

Qualifications

• RN licensure in the state of practice.
• Associate's Degree in Nursing required.
• Two years of related experience.
• Ability to communicate effectively, verbal and written.
• Familiarity with HIPAA and safety policies.

Responsibilities

• Coordinate the RAI process including MDS, Care Area Assessment, and care plan development.
• Oversee assessment completion and transmission to national repository.
• Review validation reports and correct assessments as needed.
• Coordinate comprehensive care plan per regulatory requirements.
• Maintain OBRA and PPS assessment schedules.
• Determine skilled level of care for Medicare residents and manage documentation.
• Ensure compliance with state regulations of the RAI process.
• Provide MDS-based Quality Measures insights.
• Participate in QAA and/or QAPI committees.
• Audit and improve staff education and competency.
