Job summary

Otterbein SeniorLife in Ohio seeks an experienced MDS Nurse Transition Leader to oversee MDS duties and care plan development across locations. You’ll coach staff, coordinate with coordinators, and lead Medicare/therapy meetings while upholding person-centered care.

The role requires at least 5 years of MDS experience in long-term care, a BSN, strong PDPM/casemix skills, and proficiency with EMR/PCC. Travel up to 100% may be necessary; Otterbein offers comprehensive benefits and growth

Qualifications

• BSN degree required and must be English-speaking.
• Minimum 5 years’ MDS experience in long-term care/nursing home environments.
• Strong MDS skills including casemix management and PDPM management.
• Experience with Electronic Medical Records; PCC experience highly preferred.
• Must be flexible, adaptable to change and able to communicate effectively in English.

Responsibilities

• Effectively complete the MDS and Care Plans.
• Discuss elder observations/concerns with appropriate coordinator.
• Generate/update Monthly MDS Calendar and Notification Process for Support Staff.
• Provide coaching and direction to staff as needed.
• Complete the certifications and recertifications for residents/elders.
• Facilitate the weekly Medicare/therapy Utilization Review Meetings.
• Act as the therapy liaison and provide training materials to staff.
• Collaborate in developing new processes, forms, and systems.
• Be prepared and actively participate in morning meetings.
• Assist in managing the survey process when required.
• Perform any other duties as assigned.
