Job summary

Otterbein SeniorLife is seeking an experienced MDS Nurse Transition Leader to guide MDS processes and care planning across Ohio/Indiana locations. You will mentor staff, manage PDPM/casemix tasks, and collaborate on QA initiatives within a supportive, resident-centered culture.

The role requires a BSN and at least 5 years of MDS experience in long-term care, with strong EMR/PCC familiarity and English communication skills. Travel up to 100% may be needed based on transition leadership needs.

Qualifications

• Education: BSN degree.
• Minimum 5 years MDS experience in long-term care.
• Experience with PDPM and case-mix management.
• proficiency with Electronic Medical Records; PCC preferred.
• Strong writing, literacy and interpersonal abilities.
• Must be able to communicate in English.

Responsibilities

• Effectively complete the MDS and Care Plans
• Discuss elder observations with coordinator
• Generate/update Monthly MDS Calendar and Notification Process
• Provide coaching and direction to staff
• Complete certifications/recertifications for residents
• Facilitate Medicare/therapy Utilization Review Meetings
• Act as therapy liaison
• Provide training materials to staff
• Collaborate in development of new processes, forms, systems
• Be prepared for morning meetings
• Assist in the survey process when required
• Perform other duties as assigned

This position is designed to serve as an MDS Nurse Transition Leader when an opening occurs at any Otterbein location in the MDS Nurse Position. The MDS Nurse Leader will complete the MDS Nurse duties at assigned location. When an Interim role is not needed, then the MDS Nurse Leader will be assigned special projects by the VP Clinical Quality.

Responsibilities

MDS

Qualifications

• Education: BSN
• Experience: Minimum 5 years’ of MDS experience in long-term care//nursing home environment required. Experience in post-acute environments also a plus

Special Skills and Requirements:

• Must have a proven track record of strong MDS skills (casemix management, PDPM management, Quality Measures)
• Ability to be flexible and adapt quickly to change.
• Experience with Electronic Medical Records; PCC experience highly preferred
• Must be detailed oriented and have excellent literacy and writing, documentation, communications and interpersonal abilities.
  Must be able to cope with mental and emotional stress related to working with persons who are ill, disabled, elderly and/or emotionally upset.
• Must be able to communicate in English.
