Job summary

Otterbein SeniorLife is seeking an MDS Nurse Transition Leader in Ohio. You will execute MDS duties, coordinate care plans, and lead discharge planning for residents across Otterbein locations. The role requires strong MDS expertise and RN licensure in Ohio, with potential cross-state consideration.

Travel may be required up to 100% depending on transition leadership needs. This is a senior interfacility leadership position within our MDS team, offering collaboration with nursing and quality

Qualifications

• BSN required.
• Current OH RN license; IN license a plus.
• Minimum 5 years MDS experience in long-term care.

Responsibilities

• Complete the MDS and Care Plans.
• Discuss elder observations with coordinator.
• Generate/update Monthly MDS Calendar and Notification Process for Support Staff.
• Provide coaching and direction to staff as needed.
• Discharge planning for residents including home services.
