Job summary

Eastbrook Healthcare Center in Cleveland, OH, seeks a dedicated MDS Nurse to perform comprehensive MDS assessments and support care planning in accordance with CMS and state regulations. The role collaborates with interdisciplinary teams, ensures accurate documentation, and stays updated on regulatory changes while educating staff on MDS processes.

Qualifications

• Current licensed Registered Nurse (RN) in Ohio.
• Previous experience in long-term care and familiarity with the MDS process is highly preferred.
• Strong analytical and assessment skills with a keen attention to detail.
• Excellent communication and interpersonal skills for effective collaboration with interdisciplinary teams.
• Proficient in electronic health records (EHR) and MDS software programs.
• Ability to manage time effectively and prioritize tasks in a fast-paced environment.
• Knowledge of federal and state regulations pertaining to long-term care facilities.

Responsibilities

• Conduct accurate and timely MDS assessments in compliance with CMS and state regulations.
• Collaborate with interdisciplinary teams to gather data for MDS assessments and ensure data accuracy.
• Analyze resident needs to develop care plans based on MDS results.
• Ensure MDS documentation is complete and accurate, including coding of health status and needs.
• Facilitate care plan meetings with nursing staff, social services, rehab, and other departments.
• Monitor implementation of care plans and adjust based on resident status and outcomes.
• Stay updated on regulatory changes and best practices in MDS processes and documentation.
• Educate staff on MDS processes and documentation requirements.
