Job summary

Western Hills Retirement Village is seeking an experienced MDS Nurse to conduct initial and ongoing MDS assessments, ensure regulatory compliance, and support accurate care planning.

You will collaborate with the interdisciplinary team, educate staff on MDS requirements, maintain up-to-date records, and participate in quality initiatives within a caring, family-owned environment.

Qualifications

• Graduate of an accredited School of Nursing.
• Current Ohio LPN or RN license.
• Minimum of 1 year of experience as an MDS Nurse; 3+ years in long-term care preferred.

Responsibilities

• Conduct initial and ongoing MDS assessments in accordance with federal, state, and facility regulations.
• Accurately complete all sections of the MDS, including RAI and care planning documentation.
• Collaborate with the interdisciplinary team to ensure accurate documentation of resident needs and goals.
• Provide education and guidance to staff on MDS requirements and regulatory standards.
• Ensure compliance with all federal and state regulations related to long-term care.
• Maintain accurate, up-to-date records of MDS assessments and care plans.
• Coordinate timely transmission of MDS data to appropriate agencies.
• Participate in interdisciplinary care plan meetings and provide clinical input.

Your Role as an MDS Nurse:

• Conduct initial and ongoing MDS assessments in accordance with federal, state, and facility regulations
• Accurately complete all sections of the MDS, including RAI and care planning documentation
• Collaborate with the interdisciplinary team to ensure accurate documentation of resident needs and goals
• Provide education and guidance to staff on MDS requirements and regulatory standards
• Ensure compliance with all federal and state regulations related to long-term care
• Participate in quality improvement initiatives and regulatory survey processes
• Maintain accurate, up-to-date records of MDS assessments and care plans
• Coordinate timely transmission of MDS data to appropriate agencies
• Participate in interdisciplinary care plan meetings and provide clinical input
• Monitor and update care plans based on changes in resident condition
• Maintain case-mix index at target levels
• Communicate effectively with physicians, staff, residents, families, and vendors
• Follow all facility policies, including infection control, safety, and HIPAA regulations
• Maintain a calm, supportive, and professional environment
• Attend required trainings, in-services, and meetings

What We're Looking For:

• Graduate of an accredited School of Nursing
• Current Ohio LPN or RN license
• Current CPR certification
• Minimum of 1 year of experience as an MDS Nurse required
• 3+ years of experience in long-term care or skilled nursing preferred
• Strong knowledge of federal and state long-term care regulations
• Excellent attention to detail and accuracy
• Strong organizational and time-management skills
• Effective communication and interpersonal skills
• Ability to work independently and as part of a team
• Proficiency in Microsoft Office and electronic medical records (PointClickCare preferred)
• Physically able to meet job requirements, including lifting and assisting residents as needed
