Job summary

Riverside Commons in Dearborn, MI is seeking an experienced MDS Registered Nurse (RN) to join our interdisciplinary care team. You will develop and complete resident assessments and care plans in accordance with state and federal requirements.

Responsibilities include collaborating with the team, identifying resident needs, providing education for residents and families, and participating in quality improvement initiatives to improve outcomes.

Qualifications

• Valid Registered Nurse (RN) license in Michigan.
• Minimum of 2 years of experience in a long-term care setting.
• Strong assessment and care planning skills.
• Ability to work in an interdisciplinary team.

Responsibilities

• Develop and complete resident assessments and care plans per state and federal requirements.
• Collaborate with the interdisciplinary team to implement individualized care plans.
• Identify and address resident needs and concerns.
• Provide education and support to residents and their families.
• Participate in quality improvement initiatives to improve resident outcomes.
