Job summary

Ciena Healthcare in Flint, MI is seeking an experienced MDS Nurse (RN or LPN) to join our care management team. The MDS Nurse handles the RAI process, completes assessments and care plan coordination for assigned residents, and collaborates with physicians, nursing, social services, therapy, and dietary staff.

Responsibilities include completing MDS and PPS within required timeframes, reviewing charts, coordinating care changes, and maintaining PPS boards.

Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
• Experience with MDS and PPS processes is preferred.
• Knowledge of RAI and PPS strongly preferred.

Responsibilities

• Completes the MDS, CAA's, and care plans within regulated timeframes.
• Assesses residents through physical assessment, interview, and chart review.
• Coordinates with physicians, nursing, social services, therapy, dietary, and activity staff.
