Job summary

Hancock Park Rehabilitation and Nursing Center in Quincy, MA seeks a seasoned full-time in-person MDS Coordinator to join our compassionate team. You will ensure accurate MDS/RAI processing for Medicare residents and support timely RAP completion.

The role requires an active RN or LPN license with CPR certification, a nursing degree, and prefer at least two years of MDS experience. Join our welcoming facility near public transit.

Qualifications

• Current LPN or RN state license and CPR certification required.
• Nursing degree from an accredited college or university.
• At least two years of MDS experience preferred.

Responsibilities

• Accurate completion of RAI for all Medicare patients in the facility
• Provides completion of the nursing section of all Medicare MDSs and RAPs
• Complete OBRA MDSs as PPS caseload allows

Requirements for MDS Coordinator:

• Must possess valid unencumbered, LPN or RN State License and current CPR certification
• Nursing degree from an accredited college or university
• At least two (2) years of MDS experience preferred
