Job summary

Hancock Park Rehabilitation and Nursing Center in the Quincy area is seeking a seasoned, full-time in-person MDS Coordinator to join our compassionate team and provide high-quality care to residents.

The role requires an RN or LPN license with CPR certification and at least two years of MDS experience preferred; competitive salary with a sign-on bonus and benefits are offered.

Qualifications

• Must possess valid unencumbered RN or LPN state license and current CPR certification.
• Nursing degree from an accredited college or university.
• At least two years of MDS experience preferred.

Responsibilities

• Complete accurate RAI for Medicare residents in the facility.
• Complete nursing section of all Medicare MDSs and RAPs.
• Complete OBRA MDSs as PPS caseload allows.

Requirements for MDS Coordinator

Job Duties for MDS Coordinator

• Accurate completion of RAI for all Medicare patients in the facility
• Provides completion of the nursing section of all Medicare MDSs and RAPs
