Job summary

You will coordinate MDS assessments, develop individualized care plans, and collaborate with an interdisciplinary team to ensure regulatory compliance and excellent service for our residents.

Qualifications

• Active LPN or RN license in Missouri.
• Knowledge of long-term care regulations and standards.
• Excellent communication, organizational and problem-solving abilities.
• Ability to work in a fast-paced setting and manage priorities.
• Strong analytical and critical thinking skills.
• Maintain confidentiality and discretion with resident information.

Responsibilities

• Coordinate and manage MDS assessments and care plans for residents.
• Complete and submit MDS assessments, Care Plans and required docs.
• Collaborate with interdisciplinary team to meet resident needs.
• Educate residents, families, and staff on MDS processes.
• Stay updated on regulatory requirements and best practices.
