Job summary

Qualifications

• Active LPN or RN license in Missouri.

Responsibilities

• Coordinate and manage MDS assessments and care plans for residents.
• Complete and submit MDS assessments, Care Plans and required docs.
• Collaborate with interdisciplinary team to meet resident needs.
• Educate residents, families, and staff on MDS processes.
• Stay updated on regulatory requirements and best practices.

• Coordinate and manage the assessment and care planning process for residents, including the development and implementation of Individualized Care Plans
• Complete and submit accurate and timely MDS assessments, Care Plans, and other required documentation
• Collaborate with interdisciplinary team members to identify and meet the unique needs of each resident
• Provide education and support to residents, families, and staff on the MDS process and care planning
• Stay up-to‑date with regulatory requirements and industry best practices to ensure compliance

Requirements:

• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Missouri
• Strong understanding of long‑term care services and regulations
• Excellent communication, organizational, and problem‑solving skills
• Ability to work effectively in a fast-paced environment and prioritize multiple tasks
• Strong analytical and critical thinking skills
