Job summary

Otterbein Skilled Nursing & Rehab Neighborhood seeks an ADON/MDS Nurse Coordinator to lead MDS processes and case manage care plans in a person-centered environment. You will coordinate assessments, ensure regulatory compliance, and oversee reimbursement workflows as part of the nursing leadership team.

This role requires an Ohio RN license, MDS and restorative nursing experience, and strong clinical knowledge of long-term care standards.

Qualifications

• MDS experience required; Restorative Nursing experience.
• Graduate of approved nursing program.
• Valid Ohio RN license.

Responsibilities

• Coordinate development, completion and transmission of assessments.
• Case manage care plans, including rehabilitative programs.
• Ensure knowledge of clinical standards, regulations and reimbursement governing long-term care and Medicare participation.
• Lead and deliver high quality services in a person-centered environment.
