Job summary

The Sheridan Village Nursing and Rehabilitation Center is seeking a qualified MDS Nurse/Care Plan Coordinator (LPN/RN) to ensure accurate assessments and up-to-date care plans for residents, coordinating Comprehensive Resident Assessments and reporting to the Administrator.

The role requires an Illinois license (LPN or RN), fluent English, and familiarity with MDS, EMR, and care plan documentation. Training is offered to the right candidate.

Qualifications

• Illinois license as LPN or RN is required.
• Ability to read, write and speak English clearly.
• Knowledge of MDS, EMR and care plan documentation.

Responsibilities

• Coordinate the MDS process at admission, readmission, quarterly, annual and when needed.
• Develop, update and communicate resident care plans.
• Provide in-service training on care plans and documentation.
• Monitor EMR records for accuracy and completeness.
• Communicate resident care plan status to staff, families, and administrator.
