Job summary

Big Horn Rehabilitation and Care Center in Sheridan, WY, is seeking a detail-oriented MDS Coordinator to lead the Resident Assessment Instrument process and drive accurate MDS submissions for appropriate reimbursement.

The role requires an active LPN or RN license in Wyoming, 2+ years of nursing experience (1+ year in long-term care), and strong knowledge of MDS 3.0, PDPM, and CMS regulations. A collaborative leader focused on quality care is essential.

Qualifications

• Active LPN or RN license in Wyoming
• Minimum 2 years of nursing experience
• At least 1 year in long-term care or skilled nursing (MDS experience preferred)
• Strong knowledge of MDS 3.0, PDPM, RAI guidelines, and care planning processes
• Familiarity with CMS regulations, Medicare/Medicaid billing, and quality measures

Responsibilities

• Oversee the RAI process including MDS assessments, CAAs, and care planning
• Ensure timely and accurate MDS submissions in line with CMS/state guidelines
• Monitor documentation for accurate MDS coding and reimbursement
• Lead care plan meetings with the interdisciplinary team
• Educate staff on documentation best practices and regulatory requirements
• Track resident acuity and case mix data to support planning
• Coordinate with billing, therapy, and reviewers to ensure proper reimbursement
