The Heritage Center at Holiday Health Care in Evansville, IN is seeking a dedicated Full-Time MDS Nurse to join our on-site team. This role focuses on completing MDS assessments and participating in resident care conferences to ensure high-quality care for our residents.

Qualifications

• Graduate of an accredited school of nursing.
• Active nursing license in Indiana.
• Experience in long-term care nursing (preferred).
• Knowledge of the RAI manual, ICD-10 codes, case mix index, and strong assessment skills.
• Proficient with computers and MDS submission processes.
• Strong interpersonal and communication skills.

Responsibilities

• Complete and submit MDS assessments for all nursing unit residents.
• Coordinate resident care conferences to ensure optimal care plans.
• Conduct unit rounds to assess resident function and team interaction.
• Ensure MDS is submitted on time in compliance with State and Federal guidelines.
• Collaborate with the health care team to create an interdisciplinary care plan for each resident.
• Provide education to residents, families, and staff regarding the care provided.
• Monitor the completion of ADL assessments.
• Ensure all care plans are aligned with the MDS assessments and RAP guidelines.

Key Responsibilities:
