Job summary

Scenic Wellness Nursing & Rehabilitation in Herculaneum, MO is seeking an MDS Assistant to support the Director of Nursing and RN Assessment Coordinator in documenting in full compliance with Federal, State, and Certification guidelines.

You will perform data collection, support the MDS process, and participate in the RAI workflow to ensure timely submissions and accurate patient assessments while collaborating with the interdisciplinary team.

Qualifications

• Graduate of an approved RN/LPN program and licensed in the practicing state.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding of OBRA regulations and MDS requirements.
• Knowledge of care planning process.
• Experience with the RAI process and documentation.

Responsibilities

• Assist in ensuring adherence to Federal and State regulations and certification.
• Participate in regulatory surveys and correction of deficiencies.
• Report trends from audits to the Quality Assurance Committee.
• Assist in completing the RAI Process from the MDS through the interdisciplinary completion of the plan of care.
• Initiate and monitor RAI process tracking, discharge/reentry, and Medicaid tracking forms through PointClickCare.
• Follow up with staff to assure compliance to documentation standards.
• Complete patient assessments, data collection, and interviews with staff to assure good standard of practice.
• Facilitate accurate determination of the Assessment Reference Date reflecting patient care needs.
• Ensure timely submission of the MDSs to the State with proper follow-up on validation errors.
• Maintain validation records from the submission process in a systematic and orderly fashion.

• Through understanding of the Quality Indicator process
• Knowledge of OBRA regulations and Minimum Data Set
• Knowledge of the care planning process
