Job summary

Kinder Retirement & Rehabilitation Center seeks an experienced MDS Nurse (RN or LPN) to join our skilled nursing team. This role coordinates resident assessments, supports individualized care planning, and helps ensure accurate documentation and regulatory compliance.

You will complete MDS assessments, manage the RAI process, collaborate with an interdisciplinary team, and contribute to quality-improvement initiatives while maintaining confidentiality and meeting deadlines.

Qualifications

• Current, unrestricted RN or LPN license in the state of practice.
• MDS experience in a skilled nursing or long-term care setting preferred.
• Working knowledge of MDS 3.0, RAI process, care planning, and Medicare/Medicaid requirements.
• Proficiency with electronic health records and related clinical software.

Responsibilities

• Complete and coordinate accurate, timely MDS assessments per requirements.
• Manage the Resident Assessment Instrument (RAI) process and submission timelines.
• Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members.
• Coordinate comprehensive, resident-centered care plans.
• Review clinical documentation for accuracy, completeness, and consistency.
• Monitor changes in resident condition and ensure assessments and care plans are updated accordingly.
• Support accurate reimbursement through proper documentation and coding.
• Participate in care-plan meetings, quality-improvement initiatives, audits, and regulatory surveys.
• Educate staff regarding MDS documentation and assessment requirements.
• Maintain the confidentiality of resident and facility information.

Kinder Retirement is seeking an experienced, detail-oriented MDS Nurse to join our skilled nursing team. This position plays an essential role in coordinating resident assessments, supporting individualized care planning, and helping ensure accurate documentation and regulatory compliance.

Key Responsibilities

• Complete and coordinate accurate, timely MDS assessments in accordance with federal, state, and facility requirements
• Manage the Resident Assessment Instrument (RAI) process and required submission timelines
• Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members
• Coordinate comprehensive, resident-centered care plans
• Review clinical documentation for accuracy, completeness, and consistency
• Monitor changes in resident condition and ensure assessments and care plans are updated appropriately
• Support accurate reimbursement through proper documentation and coding
• Participate in care-plan meetings, quality-improvement initiatives, audits, and regulatory surveys

Qualifications

• Working knowledge of the MDS 3.0, RAI process, care planning, and Medicare/Medicaid requirements
• RAC-CT certification preferred but not required
• Strong organizational, communication, and critical-thinking skills
• Excellent attention to detail and ability to meet required deadlines
• Proficiency with electronic health records and related clinical software
• Ability to work collaboratively as part of an interdisciplinary care team
