Job summary

Longterm Health Management Services in Illinois is seeking a dedicated MDS Nurse to join our skilled nursing facility team. The role focuses on developing, monitoring, auditing and updating each resident’s care plan, performing assessments, and assisting with discharge planning.

The successful candidate will hold an Illinois RN license, CPR certification, and bring MDS Coordinator and SNF RN/CNA experience. We offer competitive wages and opportunities for growth in a supportive environment.

Qualifications

• Must hold an unencumbered Illinois RN license.
• Graduate of an accredited nursing program and CPR certification.
• Prior MDS Coordinator experience preferred; 1 year RN SNF and 1 year CNA experience preferred.

• Perform resident assessments and participate in discharge planning.

As a MDS Nurse you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at our facility by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, an unencumbered State ofIllinoisR.N. License, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. in a Skilled Nursing Facility setting and (1) year of experience as a C.N.A. is preferable!
