Job summary

Van-Duyn-Center-For-Rehabilitation- is seeking an MDS Nurse for a full-time role with flexible hours. The position oversees federally mandated assessments and coordinates interdisciplinary care planning.

You will gather resident data, work with IDT to create care plans, and ensure timely Medicare certifications and recertifications, in compliance with Federal and State regulations. A NYS RN license is required.

Qualifications

• Current NYS RN license.
• Knowledge of MDS 3.0, CAA’s, Medicare Guidelines, and Care Planning.
• Excellent written and oral communication, including English usage, grammar, and punctuation.
• Ability to work independently and under pressure with time constraints.

The MDS Nurse is accountable for the federally mandated assessments. The incumbent will accomplish these tasks by coordinating and overseeing the interdisciplinary assessments and the care planning process.
