Reporting to nursing leadership, you will collaborate across disciplines, coordinate care planning processes, and maintain up-to-date clinical documentation with strict confidentiality and professional integrity.

Qualifications

• MDS 3.0 experience is required.
• Active NY RN license.

Responsibilities

• Complete accurate MDS assessments and comprehensive care plans.
• Initiate and oversee care planning processes and related activities.
• Develop and distribute monthly care plan schedules in a timely manner.
• Maintain and update all clinical assessments in accordance with regulations.
• Conduct ongoing audits of clinical documentation for accuracy and compliance.
• Partner with nursing leadership to address and resolve documentation issues.
• Ensure strict confidentiality of all resident and organizational information.
