Job summary

Van Duyn Center in Syracuse, NY is seeking an MDS Nurse to coordinate Federally mandated assessments and oversee the care planning process for residents on a full-time basis.

You will gather resident information, work within Federal and State timeframes, prepare Medicare certifications, and collaborate with IDT to create care plans; a current NYS RN license and strong written and oral communication are required.

Qualifications

• Current NYS RN license is required.
• Knowledge of MDS 3.0, CAA’s, Medicare Guidelines, and care planning.
• Excellent written and oral communication in English.
• Ability to work independently and under time constraints.

Overview:

The MDS Nurse is accountable for the federally mandated assessments. The incumbent will accomplish these tasks by coordinating and overseeing the interdisciplinary assessments and the care planning process.

Responsibilities:

• Gather information on facility residents for assessments.
• Work within the time frames set by Federal and State Regulations for all MDS sections.
• Assess charts and assist with communicate with IDT to create applicable care plans for residents
• Prepare and track Medicare certifications and recertifications.

Qualifications:

• Current NYS RN license
• Be knowledgeable to MDS 3.0, CAA’s, Medicare Guidelines, and Care Planning.
• Excellent written and oral communication, including English usage, grammar, and punctuation.
• Ability to work independently and work under pressure with time constraints
