This salaried, exempt role requires RN licensure in NC (LPN considered with relevant experience) and strong documentation skills.

Responsibilities include leading MDS submissions, coordinating care plan conferences, and ensuring ARD compliance with Medicare/Medicaid rules.

Qualifications

  • Active, unencumbered RN license in North Carolina.
  • LPN license considered with relevant experience.
  • MDS experience preferred.
  • Proficient with Microsoft Office Suite (Word, Excel).
  • Excellent communication and interpersonal skills.
  • Highly organized and detail oriented.
  • Ability to work independently or as part of a team.

  • Complete and submit accurate MDS assessments.
  • Oversee care plan process including conferences.

  • Participate in monthly Quality Measure meetings.
  • Work with nursing leadership to improve documentation.

Job description

University Health & Rehabilitation Center in Durham, NC is seeking a qualified candidate to join our team as a MDS Coordinator.

LPN hiring range: $37.00 - $38.50/ hr.

RN hiring range: $41.50 - $43.00/ hr.

As a MDS Coordinator your primary responsibility will be to ensure that every patient has an accurate minimum data set (MDS) assessment and work closely with the interdisciplinary team to create comprehensive plans of care after reviewing the patient’s medical record and communicating with direct care staff, the patient’s physician, and family. You will play a pivotal role in helping us meet our commitment to providing Care Beyond Compare.

What You'll Do
  • Complete and submit accurate minimum data set (MDS) assessments on patients.

What You Need
  • Active, unencumbered licensure as a Registered Nurse (RN) in North Carolina.
  • Licensed Practical Nurse (LPN) will be considered with relevant experience.
  • Skilled Nursing / Long-Term Care MDS experience preferred.
  • Proficient with computer programs including Microsoft Office Suite (Word, Excel, etc.)
  • Excellent communication and interpersonal skills.
  • Highly organized and detail oriented.
  • Ability to work independently or as part of a team.
  • This is a salaried/exempt position.
