Job summary

PACS Group is seeking an MDS Nurse (LVN/LPN) to coordinate and complete MDS assessments, ensuring accurate documentation and adherence to CMS and state guidelines. You will work with interdisciplinary teams to promote quality outcomes and assist with reimbursement processes.

Ideal candidates hold an active LVN/LPN license in the applicable state, have experience in long‑term care, and are proficient with MDS 3.0/RAI processes and EHR systems.

Qualifications

• Must hold an active LVN/LPN license in the state of practice.
• Prior long‑term care or skilled nursing experience preferred.
• Strong knowledge of MDS 3.0, RAI, and Medicare/Medicaid guidelines.
• Excellent organizational skills with attention to accuracy.
• Ability to collaborate with interdisciplinary teams and document diligently.

Responsibilities

• Complete MDS assessments and CAAs within required timelines.
• Ensure accuracy of resident documentation to support care and reimbursement.
• Maintain compliance with CMS, state, and facility guidelines.
• Collaborate with interdisciplinary teams to develop individualized care plans.
• Monitor resident progress and update care plans as needed.
• Participate in care conferences and quality improvement initiatives.
