PACS Group seeks a licensed LVN/LPN to coordinate and complete MDS assessments, ensuring compliant documentation for Medicare/Medicaid. The role collaborates with nursing, therapy, social services, and dietary teams to support quality outcomes in a long-term care setting.

Ideal candidates will have MDS 3.0/RAI knowledge, strong organization, and experience with EHR systems. Excellent communication with residents and families is essential, with emphasis on accurate reporting and staying current

Formación

• Current LVN/LPN license in the state of practice.
• Knowledge of MDS 3.0, RAI process, and Medicare/Medicaid guidelines.
• Strong organizational skills with attention to detail.
• Ability to collaborate with interdisciplinary teams and use EHR systems.

Responsabilidades

• Coordinate and complete MDS assessments and CAAs within timelines.
• Ensure accuracy of resident documentation to support care planning.
• Maintain CMS/state compliance and CMS/Medi-Cal reimbursement readiness.
• Collaborate with interdisciplinary teams to develop individualized care plans.
• Monitor resident progress and update care plans as needed.
• Participate in care conferences and quality improvement initiatives.
• Assist with audits and surveys with accurate documentation.
• Stay current with regulatory changes affecting MDS and documentation.
• Communicate with residents and families regarding care planning.
