Job summary

The Laurels of Kent, part of Ciena Healthcare, is seeking an MDS Nurse to assist with timely completion of the Resident Assessment Instrument (RAI), including MDS, CAAs, care planning and supporting documentation in line with regulations. The role supports individualized care planning and accurate assessments.

Qualifications

• Knowledge of RAI process, PDPM, Quality Measures and Care Area Assessments.
• Strong assessment skills and ability to work independently.
• Demonstrated leadership and coordination with IDT.

Responsibilities

• Coordinate scheduling of the RAI process with IDT.
• Complete MDS, CAAs and care plans within regulatory timeframes.
• Gather timely assessment data to ensure MDS completion.
• Ensure compliance with PDPM and Case Mix guidelines.
• Participate in PPS meetings and maintain logs.
