Laurels of Kent in Lowell, MI is seeking an experienced MDS Nurse (RN) to lead RAI processes and ensure timely MDS/CAA completion. The role requires strong knowledge of RAI, PDPM, and care planning, with leadership in an interdisciplinary team and compliance with federal/state guidelines.

You will coordinate with staff and family, participate in PPS meetings, and support quality improvement initiatives while maintaining regulatory timelines.

Qualifications

• RN with one year of experience preferred.
• Knowledge of RAI guidelines, PDPM, ICD-10 coding, quality measures and care planning.
• Strong independent functioning and leadership ability.

Responsabilités

• Coordinates scheduling the RAI process with the interdisciplinary team.
• Completes the MDS, CAAs, and care plans within regulatory timeframes.
• Gathers timely, accurate assessment data with IDT for MDS completion.
• Ensures compliance with PDPM and Case Mix payment systems.
• Communicates changes in resident care needs to the IDT.
• Coordinates PPS meetings and maintains PPS log for monitoring Medicare days.
• Participates in resident care activities as directed, and supports care conferences.
