Job summary

The Laurels of Kent is seeking an experienced MDS Nurse to manage the RAI process, complete MDS assessments, and coordinate care planning for assigned residents. This role emphasizes autonomy, organization, and collaboration with the care team in a skilled nursing environment.

Qualifications

• RN or LPN license required.
• Knowledge of RAI and PPS preferred.
• Experience as an MDS Nurse.

Responsibilities

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies for assessment and care planning.
• Coordinates and initiates significant change MDSs and PPS meetings.
