Job summary

The Laurels of Coldwater is seeking an MDS Nurse to manage the Resident Assessment Instrument process, including the MDS, CAAs, and care planning for residents. You will work with the interdisciplinary team to ensure accurate data, regulatory alignment, and timely documentation, contributing to high-quality resident care in a Michigan facility.

Qualifications

• Prior MDS/RAI experience preferred.
• Knowledge of RAI, PDPM, ICD-10, Quality Measures.
• Regulatory standards and compliance knowledge.

Responsibilities

• Coordinate scheduling the RAI process with the interdisciplinary team.
• Complete the MDS, CAAs, and care plans within regulatory timeframes.
• Coordinate with interdisciplinary team to gather timely, accurate assessment data for MDS.
• Ensure compliance with PDPM and Case Mix payment systems.
• Communicate changes in resident care needs with the team.
• Coordinate and initiate significant change MDSs.
• Conduct thorough resident assessments during quarterly reviews.
• Maintain PPS log and PPS meetings per Medicare schedule.
• Assist in resident care activities as directed by supervisor.
• Coordinate Resident Care Conferences with Social Worker for care planning.
