Job summary

The Laurels of Coldwater is seeking a skilled MDS Nurse to assist with timely completion of the MDS, CAAs and care planning, ensuring assessments reflect residents’ clinical status. This role supports individualized care planning and regulatory compliance in a skilled nursing environment.

Qualified candidates will have LPN licensure with at least one year of assessment experience and CPR certification. Excellent assessment skills and independence are preferred.

Qualifications

• Prior MDS/RAI experience strongly preferred.
• Knowledge of RAI guidelines, PDPM, ICD-10 coding, Quality Measures, and Care Area Assessments and Care Planning processes.
• Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS) and Case Mix systems strongly preferred.
• Knowledge of regulatory standards and compliance requirements.
• Demonstrated ability in independent functioning and strong leadership.

Responsibilities

The MDS Nurse is responsible for assisting with the accurate and timely completion of the Resident Assessment Instrument (RAI), including the Minimum Data Set (MDS), Care Area Assessments (CAAs), care planning and supporting documentation in accordance with federal and state regulations. This role ensures assessments reflect each resident’s clinical status and supports individualized care planning.

Education and/or Experience:

• Prior MDS/RAI experience strongly preferred
