• Active Florida RN or LPN license.
• Minimum 2 years as MDS Coordinator/Director in a SNF.
• Strong knowledge of MDS 3.0 and PDPM.
• Familiar with Medicare regulations and reimbursement.
• Experience with EMR in long-term care.
• Proven leadership and team management.

Aufgaben

• Oversee accuracy and timeliness of all MDS assessments and documentation.
• Ensure PDPM compliance and regulatory standards.
• Conduct comprehensive resident assessments and interviews.
• Develop and monitor individualized care plans for residents.
• Lead interdisciplinary care plan meetings.
• Collaborate with nursing staff, physicians, therapists, residents, and families to improve outcomes.
• Provide education to staff on MDS processes and documentation standards.
• Train staff on MDS 3.0, RAPS, CAAs, PDPM, and Medicare guidelines.
• Manage MDS schedules and care plan timelines.
• Monitor reimbursement opportunities while ensuring compliance.
• Support quality improvement initiatives related to documentation and outcomes.
