Qualifications

• Active Florida RN license.
• Minimum 2 years of MDS Coordinator experience in a Skilled Nursing Facility.
• Strong knowledge of MDS 3.0, PDPM, Medicare regulations, and care planning processes.
• Excellent leadership, organizational, and communication skills.

Responsibilities

• Oversee accuracy, completeness, and timeliness of all MDS assessments and documentation.
• Ensure compliance with Medicare PDPM requirements.
• Conduct comprehensive resident assessments and interviews.
• Develop and monitor individualized care plans for short- and long-term residents.
• Lead and participate in interdisciplinary care plan meetings.
• Collaborate with nursing staff, physicians, therapists, and families to improve outcomes.
• Provide leadership and ongoing education to nursing staff.
• Conduct training on MDS, RAPS, triggers, PDPM, and Medicare guidelines.
• Manage MDS and care plan scheduling for all residents.
