Job summary

Victoria Crossing Rehabilitation Center in Brandon, Florida seeks an experienced RN/LPN MDS Coordinator to oversee the RAI/MDS process and ensure regulatory compliant, individualized care plans for residents. The role requires coordinating assessments, maintaining documentation, and supporting interdisciplinary teams to improve quality outcomes.

The position is full-time, Monday–Friday, 8:00am–4:30pm, with compensation between $38.00 and $44.00 per hour.

Qualifications

• Current Florida RN or LPN license.
• Minimum 2 years in long-term care or skilled nursing preferred.
• MDS experience strongly preferred.
• Strong knowledge of RAI/MDS regulations and reimbursement.
• Excellent communication, organizational, and time-management skills.

Responsibilities

• Coordinate and oversee MDS assessments per regulations.
• Ensure timely and accurate submission of MDS docs.
• Develop and maintain MDS-related policies and care planning.
• Complete medical forms, reports, evaluations, and documentation.
• Monitor QA/QI indicators and take corrective actions.
• Facilitate interdisciplinary care plan meetings and resident-centered planning.
• Collaborate on discharge planning and audits/inspections.
