Beach Breeze Rehab and Care in West Palm Beach, FL seeks an experienced MDS Director (RN) to lead the MDS program, ensure accuracy, and optimize clinical documentation for reimbursement. This leadership role collaborates across disciplines to support resident-centered care and regulatory compliance.

The ideal candidate brings 2+ years of MDS Coordinator experience in a SNF, strong PDPM/Medicare knowledge, and proven leadership to drive care planning and staff development.

Qualifications

• Active, unencumbered Florida RN license.
• Minimum 2 years of MDS Coordinator experience in a Skilled Nursing Facility.
• Strong knowledge of MDS 3.0, PDPM and Medicare regulations.
• Proven leadership and ability to manage deadlines in a fast-paced setting.
• Excellent organizational and communication skills.

Responsibilities

• Oversee accuracy, completeness, and timeliness of all MDS assessments.
• Ensure compliance with Medicare PDPM requirements.
• Conduct resident assessments and interviews.
• Develop and monitor individualized care plans for residents.
• Lead interdisciplinary care plan meetings.
• Collaborate with nursing staff, physicians, therapists, residents and families to improve outcomes.
• Provide leadership and ongoing education to nursing staff.
• Conduct in-service training on MDS, RAPS, triggers, PDPM, and Medicare guidelines.
• Maintain and manage MDS and care plan scheduling for all residents.
