Qualifications

• Registered Nurse with required current state licensure.
• Minimum three (3) years of clinical experience in a health care setting; long-term care setting preferred.
• Minimum one (1) year of MDS experience.

Responsibilities

• Collect information to complete the MDS using medical records and assessments.
• Participate in Daily PPS meetings and review Plan of Care quarterly.
• Provide education on the RAI process to all team members.
