Job summary

Signature HealthCARE at North Hardin Rehab & Wellness Center is seeking an MDS-focused Registered Nurse to lead accurate resident assessments and MDS documentation. The role involves bedside evaluation, family interviews, and collaboration with an interdisciplinary team to ensure high-quality care and compliant billing.

The ideal candidate will have a current RN license, robust MDS experience, and a commitment to person-centered care in a long-term care setting.

Qualifications

• Registered Nurse with required current state licensure.
• Experience with MDS processes and resident assessments preferred.
• Strong communication and collaboration with interdisciplinary teams.

Responsibilities

• Collect information to complete the MDS using medical records, bedside assessments, and interviews with staff, residents and families.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month-end reviews to ensure billing accuracy.
• Educate the care team on RAI/MDS processes and changes across the quarter.
• Support and mentor staff on MDS workflows and regulatory requirements.
