Job summary

Signature HealthCARE at North Hardin Rehab & Wellness Center in Radcliff, KY is seeking an MDS Candidate to join our resident-centered team. You will collect information for the MDS, perform bedside assessments, and participate in daily PPS and quarterly plan reviews to support compliant federal billing.

The ideal candidate is detail-oriented, organized, and able to communicate with families and interdisciplinary staff.

Qualifications

• RN licensed in Kentucky and in good standing.
• Minimum three years of clinical experience; long-term care preferred.
• Minimum one year of MDS experience.

Responsibilities

• Collect information to complete the MDS using the medical record, bedside assessment, and staff/family interviews.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month-end reviews for accurate billing.
• Educate the RAI process to the interdisciplinary team and caregivers.
