Job summary

Essex Meadows is seeking an experienced MDS Coordinator (RN) to oversee twenty-four-hour nursing care, ensuring a compassionate, resident-centered approach and adherence to quality standards.

You will lead the MDS process, coordinate care planning with Nursing, Therapy, Social Services, Dietary, and Activities, optimize PDPM reimbursement, and ensure CMS compliance.

The role requires regulatory knowledge and supervisory experience to support a collaborative, mission-driven team.

Qualifications

• CT RN license and BLS certification required.
• Minimum 6 months nursing experience in long-term care preferred.
• In-depth experience with MDS software and regulatory requirements.

Responsibilities

• Coordinate interdisciplinary care planning with Nursing, Therapy, Social Services, Dietary, and Activities.
• Lead IDT meetings including Daily PDPM, Utilization Review, Monthly Triple Check, and Care Plan Conferences.
• Submit accurate, timely MDS assessments per CMS and regulations.
• Select ARDs and diagnosis codes for PDPM classification and reimbursement.
• Monitor PDPM components and ensure CMS compliance.
• On-call leadership rotation to support the team.
• Perform other duties as assigned.
