Job summary

Heritage Park Health Center by Harborview is seeking an experienced MDS Coordinator (RN) to lead clinical assessment and reimbursement processes. You will ensure accurate MDS documentation, drive optimal reimbursement, and comply with CMS requirements while collaborating with the interdisciplinary team to develop person-centered care plans.

In this role, you will coordinate care planning across nursing, therapy, social services, dietary, and activities, and participate in IDT meetings focusing

Qualifications

• Active RN license in good standing in the practicing state.
• Experience in skilled nursing and long term care settings.
• Experience with MDS tracking and assessment tools is a plus.

Responsibilities

• Complete and submit timely, accurate MDS assessments.
• Select ARDs and diagnosis codes to ensure PDPM classification and reimbursement.
• Monitor PDPM components and ensure CMS compliance.
• Coordinate and lead care planning across Nursing, Therapy, Social Services, Diet, and Activities.
• Participate in IDT meetings including PDPM, CMI, Utilization Review, Triple Check, and care plan conferences.
• Utilize PCC, SimpleLTC, NetHealth, Teams, IQIES, Myers & Stauffer, Power DMS, and Google Sheets.
• Perform other duties as assigned.
