Fairview Skilled Nursing Home is seeking an experienced MDS Coordinator RN to oversee the MDS/RAI process, ensure timely and accurate assessments, maintain regulatory compliance, and support quality resident care and reimbursement.

The role requires current New York State RN licensure and MDS 3.0/PDPM knowledge, with strong organizational and communication skills to collaborate with the interdisciplinary team.

Qualifications

• Current New York State RN license.
• MDS 3.0, PDPM, Medicare/Medicaid knowledge.
• Strong organizational and documentation skills.

Responsibilities

• Complete and coordinate MDS, CAAs, and care plans.
• Ensure CMS/Medicare/Medicaid/PDPM/OBRA compliance.
• Monitor assessment schedules and deadlines.
• Collaborate with the interdisciplinary team on resident assessments and care planning.
• Review documentation for coding accuracy and reimbursement.
• Monitor Quality Measures and assist with QAPI initiatives.
• Support survey readiness and educate staff on MDS documentation.
