Job summary

Fairview Skilled Nursing Home is seeking a detail-oriented MDS Assessor RN to complete resident assessments and ensure accurate MDS documentation. You will support regulatory compliance and contribute to high-quality resident care.

Responsibilities include gathering assessment information from residents, families, and the interdisciplinary team, collaborating with nursing, therapy, social work, and other departments, and maintaining CMS, Medicare, Medicaid, and PDPM guidelines adherence.

Qualifications

• Current New York State RN license.
• Skilled nursing or long-term care experience preferred.
• Knowledge of MDS 3.0, PDPM, Medicare, and Medicaid.
• Strong assessment, organizational, and communication skills.

Responsibilities

• Complete MDS assessments accurately and on time.
• Gather assessment data from residents, families, and the inter-disciplinary team.
• Ensure documentation supports MDS coding and regulatory requirements.
• Collaborate with nursing, therapy, social work, and other departments.
• Maintain CMS, Medicare, Medicaid, and PDPM compliance.
• Assist with care planning and quality improvement initiatives.
